Please complete our online application. Incomplete information will delay the processing of your work history or prevent it from being submitted.

This process will take 20–30 minutes

To fill out this application, you will need to know the following:
  • Social Security Number
  • Home address history for the past 3 years
  • Current drivers license number
  • Employment/Work history up to 10 years
  • History of traffic accidents, violations and/or convictions from the last 5 years (reckless driving convictions, and license suspensions)
To qualify with Oakley Trucking, you must meet the following criteria:
  • Must be at least 23 years of age
  • Must have at least 2 years of verifiable over the road experience
  • Class A CDL with HazMat (H) or combination (X) endorsement
  • No preventable accidents involving a fatality, bodily injury or disabling damage to a motor vehicle within three (3) years
  • No more than four (4) moving convictions in the past 36 months and no more than two (2) moving convictions in the previous 12 months
  • No DUI’s in the past ten (10) years

  • Please complete all fields in each section.
  • You must complete all required fields before you can continue on to the next section.
  • When you are done with a section press “Save”. The system will alert you if there are required fields that must be complete before saving or continuing on to next section.
  • Upon completing and saving the “Registration” page, you may return to the application at a later time. Enter your first name, last name and the last four digits of your social security number on the “Welcome” page, and you will be returned to your incomplete application. Any information that was saved will be available when you return to the application.
  • You can navigate through the application by clicking a tab at the top or the “Next” button located in each section.
  • Signature Requirement: It is strongly suggested you provide an electronic signature at the end of the application process. You will be asked throughout the application to apply your signature to multiple forms by clicking a check box. If you choose not to sign electronically, you will be emailed or mailed release forms to sign and return to the application — which will delay the processing time.
